EHS Topic of the Month: Lockout/Tagout Annual Certification

The Occupational Safety & Health Administration (OSHA) requires annual certification of all lockout/tagout procedures. (This is different than the employee training required for each procedure!)

The following form may be used to record the annual certification of your lockout/tagout procedures. Items in blue italics must be completed for your facility, and all certification reports must be signed by an individual and dated. One certification form is required per procedure, not for each employee.
